The calculator ships with 77 plan rows from the verified dataset — compounded and brand, both medications — each carrying its advertised rate, all-in monthly cost, first payment, and 3/6/12-month totals exactly as the index computes them, with its evidence grade and verification date. A custom mode applies the same published formulas (all-in = medication + mandatory fees; 28-day billing × 365.25/28/12 for the calendar-month equivalent) to numbers you type.

Pick a medication and a plan. Every figure shown is the number already published in the tables — computed at build time from the dataset, not recomputed in your browser — so the calculator can never disagree with the index.

For a plan we have not verified: enter the numbers from the provider’s page and this applies the site’s published formulas — nothing else. It cannot model dose surcharges, promos or prepayments; the verified rows above do.

Where do the calculator’s numbers come from?
From the same build that generates every table: the verified rows are precomputed server-side from the dataset, so the calculator and the index cannot disagree. The custom mode applies the published formulas to your inputs and models nothing it cannot see.
Why does a prepaid plan show the same total at 3, 6 and 12 months?
Because that is what it costs to be on it at all — the prepayment is spent whether you stay three months or twelve. The first-payment row is where prepaid and monthly plans differ most.
